Be prepared to share any recent lab results or relevant clinical reports when submitting the digital intake form, as Dr. Clark will review everything at the same time.

We will confirm when the digital Case Review submission (and any labs/records) is received.
Google users:
When signed in to your Google or GMAIL account, your progress will be automatically saved while completing the digital intake form. Please note, we cannot see forms in progress or recover partially-completed responses.
Non-Google users:
After starting the digital intake form, do not close the webpage—your progress will be lost if you are not signed in under a Google account.
